Procedures for moving

After registering your new address, you are requested to follow the procedures for changes in status for each of the welfare services that you are using. You should contact such bureaus as the bureau in charge of welfare for the disabled at the municipality where you have registered your new address.
If you have asked social workers or others to conduct procedures on your behalf, you should give them your new address, and make sure that they conduct the required procedures.

Physical disability certificate
Procedures	You are requested to follow the procedures for changing your address on the certificate at the municipality of your new address.

Payment for medical services and support for persons with disabilities (medical rehabilitation services)
Procedures	There are two cases: No.1, the municipality of your new address continues the payment for medical services and support for persons with disabilities that you received from your old municipality, just by registering the change of address. No.2, you are requested to apply for a new registration. As the procedures vary depending on the circumstance of each person, please contact your municipality for details. You are also requested to change the registration of your designated medical institution in cases where institution that you are receiving medical treatment from and/or the pharmacy to fill your prescription changes.

Medical support for the severely disabled (welfare medical services)
Procedures	A new certificate will be issued by the municipality that you move to. As medical support for the severely disabled (welfare medical services) is operated by separate municipalities, income limits and content of benefits may vary. Please inquire at the bureau in charge.
Required items	Health insurance card My Number Card Tax certificate (Please contact personnel in charge of welfare for the disabled to confirm which year's certificate is required)
